All of these forms need to be completed in their entirety for the mediation to be scheduled. EACH DISPUTANT MUST COMPLETE THEIR OWN DISPUTANT INFORMATION FORM.
This form should be completed by the person requesting the mediation. This may be a disputant or another person such as a hall director or student advisor.
This form needs to be completed by all the disputants who will be involved in the mediation. The mediation cannot be scheduled without this form because it will contain when the mediation can be held.
This form should be completed by those who have gone through a mediation. It helps us in improving our mediation training and the process we use. It should only take a few minutes and is completely anonymous.
